Subject: Fatality Accident Date: 04/29/2026 Location: Highway 275 and 541st Avenue On April 29th, 2026, at approximately 4:18 a.m., a double fatality accident occurred on Highway 275 and 541st Avenue, in Madison County 2 miles East of Meadow Grove. Through an investigation conducted on the scene, it was determined that a Blue 2018 Honda Odyssey traveling westbound on Highway 275 crossed over the center line. This caused a head on collision with a 2026 Peterbilt Tractor Truck hauling an empty tank trailer traveling Eastbound. The Honda spun around and came to rest facing Eastbound in the Westbound Lane. The Peterbilt came to rest facing Eastbound in a field on the Southside of Highway 275. The driver of the Honda Odyssey, 51-year-old Lynn Phelps of Hastings, OK, and the passenger of the Honda Odyssey, 51-year-old Robert Phelps of Hastings, OK, were pronounced deceased on the scene. Next of kin has been notified. The driver of the Peterbilt Tractor Truck , 71-year-old Michael Klasna of Emmet, NE was transported to Faith Health to be treated for injuries. Seatbelts were worn by all involved. Agencies involved were; Madison County Sheriff’s Office, Nebraska State Patrol, Meadow Grove Fire and Rescue, and Battle Creek Fire and Rescue.
